Advanced Directives
An important way to maintain control over your care is to appoint
someone you trust to make health care decisions for you if you lose the ability
to make decisions. You can find these "heath care proxy" forms at
hospitals and doctors' offices or online through the New
York State Department of Health.
You can also obtain the form locally from the The Health Planning Council
